Financial Assistance Form

Please complete this form to be considered for financial assistance for your player/players. MBA strives to provide each kid the opportunity to play basketball regardless of a families ability to pay. Please complete this form in its entirety for consideration. An MBA Board member will reach out to you once the application has been received. Please note, we are now strongly urging applicants to submit one of the following documents to prove financial need. MBA has limited funds for scholarships so we need to make sure that those kids who need it the most are receiving aid. Please email one of these options to Kyle@mbahoops.org. Please know your personal information will only be viewed by our financial assistance committee and will not be printed in any form. 

After completing this form, please send one of the following as proof of need for financial assistance. 

1. Proof of free/reduced school lunch

2. First page of most recent Federal tax return.

If you do not wish to provide either, we will still consider your request however these documents will greatly help our determination of need.
